How do shinobi become jounin?

I've heard of a jounin exam from somewhere but I don't know if its canon. Since chuunin can't take the chuunin Exam to become jounin then how else will they ascend to jounin rank? Are they appointed and if so can a genin ascend to jounin because of their skill?

Evaluation reports by The Kages and Feudal Lords Temari,Kankaru and Gaara when they took the Chunin Exam with Hinata,Sakura and the rest were promoted to Jonin then and their the rest to Chunin.
