Neutron stars are usually very small. Neutron stars get their names because it's core is just densely pack neutrons. Pulsars, for instance, can be as small as 10 miles in circumference, but still have the gravity to tear a red giant to shreds.

Of course Nagato's isn't on Madara's level. The size of a neutron star is smaller than Madara's CT and the mass of the black chakra is magic don't need to explain that part.

It's very likely to, but won't always happen. I'm not an astrophysicist so it's hard to explain. But basically, the strong forces that hold an atom together, hold the neutron star together.
